What Insurance
Providers Are Accepted at
Avant Physical Therapy?
Some Regence and Premera plans now require pre-authorization for physical therapy services after the initial evaluation. This process is completed through a company called EviCore. Regardless of how many visits your referring physician has authorized or how many visits are available through your insurance plan, EviCore will still need to authorize all care in PT. After an initial set of typically 6-8 authorized visits have been utilized, Avant PT will submit all necessary paperwork to Evicore on your behalf to request further PT visits if they are medically necessary. We will let you know after benefits are checked if this is required for your particular benefit package.
